  • Review & answer each section carefully– your responses will be used to determine the treatments that are most appropriate for you.
  • Any omissions could result in a delay of your treatment.
  • New client appointments are scheduled for a longer period than current client appointments. If you are a new client and book a current client appointment, we will need to reschedule you.

A credit card will be required to book an appointment. The card is only held on file and will not be charged until your appointment.

In the event of: Cancellations with less than two business day’s notice, late arrivals (15+ min) that result in cancellation, or no-shows, a $100 fee will be charged to the card on file.

Happiness & Confidence with Allure

If you are having trouble booking online, please call our office at 610 393 1253 for assistance. We are located at 150 Allendale Rd. Suite 2100, King of Prussia, Pennsylvania 19406. It is about 20 minutes from Philadelphia, PA & West Chester, PA, 1 hour from Allentown, PA and approximately 2 hours from Wilmington, DE.

New to Allure Clients

  1. Each Medical Condition will automatically be checked “No,’ so be sure to check “Yes” to anything that may apply.
  2. List any allergies you have and any medications you are currently taking.
  3. We must have a listed medical condition for any medications you are currently taking (other than birth control) or it will have to be confirmed before you receive treatment (this includes medications being taken for depression, anxiety, etc.) and may require rescheduling your appointment.
  4. If you are taking medications without a diagnosed medical condition, please select yes to the option ”If taking medications for a reason not listed…” under the Medical Conditions section and type the reason for your medication(s) in the comment box.
  5. A credit card will be required for booking your first appointment. This card will be charged $150 for your deposit within 1-2 business days. Please review the appointment/deposit policies agreement on our portal for more information.
