  • Before booking every appointment, please login to our portal and review your medical information on file.
  • Remember to update your information each time if anything has changed.
  • If you did not rebook at your last appointment, please call or email the office directly instead of booking online.

Booking Policies

Credit Card

A credit card will be required to book an appointment. The card is only held on file and will not be charged until your appointment.

In the event of: Cancellations with less than two business day’s notice, late arrivals (15+ min), or no-shows, a $100 fee will be charged to the card on file.

Feeling Ill

Due to the hygienic nature of our services and office, we cannot accept sick patients! If you are feeling ill, please call or email as soon as possible to reschedule.
